Mendon issued citations in death of village worker

MENDON - The village received four citations stemming from the death of a village employee in February.
Ohio Bureau of Workers' Compensation officials after an investigation reported four conditions that did not comply with Ohio Public Employment Risk Reduction Program standards. In each of the four conditions, the violations were corrected during the inspection, thus no other penalty was imposed against the village.
The village was cited for,
• not establishing a program for servicing and maintaining the dump truck salt auger/spreader.
• allowing an employee to perform maintenance on the salt auger/spreader while it was still running.
• leaving the equipment with a 71/2-inch opening unguarded.
• not maintaining incident report forms according to instructions on the forms.
Ohio BWC Public Relations Manager Melissa Vince confirmed that all issue detailed in the citations have been corrected. If an employer would fail to comply with the results of an investigation, a local court could impose a penalty of up to $500 per day for each violation.
On Feb. 16, Brendan Poling, 49, was killed in an industrial accident at the village's water-treatment facility. He reportedly had climbed into the bed of a dump truck filled with salt and had slipped into the salt auger.
